AAPHP has been asked to develop a one page sheet for physicians to adapt to their practice in informing patients about COVID-19 vaccine facts and myths.   The most recent version is here: FINAL DRAFT 10Jan21... In addition, everyone who is able to receive the vaccine should be provided with the appropriate up-to-date  EUA or vaccine information sheet for that vaccine and assessed for contraindications.  The official fact sheets for each vaccine can be obtained from  https://www.cdc.gov/vaccines/covid-19/eua/index.html as well as   here  www.modernatx.com/covid19vaccine-eua and here     https://www.cvdvaccine.com/

Providers are also reminded of the need to report adverse vaccine events to the Vaccine Adverse Event Reporting System (VAERS): https://vaers.hhs.gov/reportevent.html
